SPRINGFIELD, Mo. -- It was a year ago that two people were killed in a six-vehicle crash on one of Springfield's busiest streets. Now, a woman from Ava is charged with causing the accident. Prosecutors say Tammy Thompson was changing lanes along West Bypass on May 19, 2012 when she clipped another vehicle, sending it into the opposite lanes where it hit a motorcycle head-on. It immediately killed the couple on the bike - Gary and Denice Zenger of Bois D'Arc. If convicted of the misdemeanor traffic charge, Thompson could face up to a year in jail.

FORSYTH, Mo -- There is a disturbing trend happening across the country--more teens admit to texting and driving. Monday, Forsyth High School showed students exactly what can happen when you aren't paying attention behind the wheel. It was only a few years ago that mock accidents were shown to let students know the dangers of drinking and driving. While that is certainly still a very serious issue, it's now texting and driving that causes more deaths every year. "Once I walked up there and saw my friends, who I've known for several years, actually look like that, it was hard," said mock accident participant and Forsyth student Breeana House.

ALTUS, Okla. - A worker killed in an industrial accident in Oklahoma has been identified as a man from Springfield. Authorities say 57-year old Mark Rooney died in the accident on Tuesday afternoon at the Planters Cooperative grain elevator in Altus. Rooney had been working inside a lift basket when a co-worker found him. Authorities say it appears Rooney was pinned by a beam. He was pronounced dead at the scene. The federal Occupational Safety and Health Administration, or OSHA, will investigate.

BRANSON, Mo. -- A crane tipped over on Monday morning, creating quite a mess in downtown. The crane was lifting a piece of refrigeration equipment at Mr. Ice Man, a business near the Branson Scenic Railway depot. Branson Fire Chief Ted Martin said the crane fell into some power lines about 8:15 a.m., knocking out power to about a 10-block area. By midday, only about one block didn't have power. Martin said a small amount of refrigerant might have leaked from the equipment that the crane was trying to lift, so firefighters asked people in the immediate area to stay indoors as a precaution.

SHANNON COUNTY, Mo. --One man is dead after his vehicle rolled near Mountain View around 5 a.m. on Saturday. Dylan Sansouc's vehicle ran off T Highway and overturned several times, according to the Missouri State Highway Patrol. Troopers said Sancouc, 20, was ejected from the vehicle and pronounced dead at the scene. No word on why the vehicle swerved off the pavement.

LEBANON, Mo.-- A 25 year old Lebanon woman died Friday night as the result of a one vehicle accident. Tosha Odien was killed when the 1998 Ford F150 pickup she was driving ran off the right side of the roadway and overturned. The accident happened on Highway MM seven miles east of Lebanon. The Missouri State Highway Patrol says Odien was not wearing a seat belt and was ejected. She was pronounced dead at the scene.

ST. LOUIS, Mo. -- Tyson Foods agreed to pay nearly $4 million in civil penalties to settle violations of the Clean Air Act after a series of chemical accidents at its facilities, the Justice Department and Environmental Protection Agency announced Friday. The settlement was filed in U.S. District Court in St. Louis, with Tyson agreeing to conduct testing and inspections of its ammonia refrigeration systems at all 23 of the company's facilities in the Midwest. Tyson operations in Missouri that would be affected include plants at Monett, Noel and Sedalia.

SPRINGFIELD, Mo. -- The 7 a.m. shift on Wednesday, Jan. 30, began like most at the 911 dispatch center in the middle of an Ozarks winter - calm and essentially uneventful. “The biggest problems we thought we would face that morning, in addition to routine service calls, included getting a quick break for breakfast and making sure someone made coffee,” said Springfield-Greene County 911 telecommunicator Daphne Dauzat. Three minutes into the shift, everything changed. Seven-year-old Kailee Mangan called 911 from a cell phone.

SPRINGFIELD, Mo. -- A five-vehicle crash shut down National Avenue in all directions, at its intersection with Catalpa Street, around 4 p.m. Saturday. The cause of the crash is still under investigation. Four people were taken to hospitals; one is a small child. Police say one person is in critical condition. Police say a car driving north on National struck another car in the turning lane. That's when the first car went into oncoming traffic, hitting three southbound cars.

SPRINGFIELD, Mo. -- The City of Springfield is being sued by a couple from Polk County whose son died after police used an electric stun gun on him. Waldo and Neoima Hawkins seek $750,000. The lawsuit is based on an incident that occurred in December 2009 when police arrested Michael Hawkins for suspected burglary and resisting arrest at the Eagles Lodge Motel on North Glenstone Avenue. During the encounter, Springfield police deployed a Taser on Hawkins while he was hunkered down in a crawl space at the motel.
